چکیده

Donation after circulatory death (DCD) grafts are commonly used in liver transplantation. Attributable to the additional ischemic event during donor warm ischemia time (DWIT), DCD carry an increased risk for severe ischemia/reperfusion injury and postoperative complications, such as cholangiopathy. The actual DWIT depends on course of vital parameters withdrawal life support varies widely between donors. period (functional DWIT) starts when either Spo2 or blood pressure drop below a certain point lasts until start cold perfusion organ retrieval. Over years, multiple definitions thresholds functional duration have been used. International Liver Transplantation Society organized Consensus Conference DCD, Preservation, Machine Perfusion January 31, 2020 Venice, Italy. aim this conference was reach consensus about various aspects transplantation context currently available evidence. Here we present recommendations with regards DWIT, importance support, acceptable proceed

Hypothermic Machine Perfusion Preservation of the DCD Kidney: Machine Effects

Purpose. Kidneys from DCD donors represent a significant pool, but preservation problems exist. The study objective was to test the importance of machine type for hypothermic preservation of DCD kidneys. Methods. Adult Beagle dog kidneys underwent 45 minutes of warm in situ ischemia followed by hypothermic perfusion for 24 hours (Belzer-MPS Solution) on either an ORS LifePort or a Waters RM3 us...

Warm ischemia may damage peribiliary vascular plexus during DCD liver transplantation.

Biliary complications cause significant morbidity and mortality in liver transplantation. Warm ischemia can induce biliary duct injury. This study aimed to investigate the effects of warm ischemia on the peribiliary vascular plexus in rat liver transplantation. A total of 102 Sprague-Dawley rats were divided into three groups: sham-operation group, non-ischemic group, and ischemic group. Liver ...

Liver Graft Susceptibility during Static Cold Storage and Dynamic Machine Perfusion: DCD versus Fatty Livers

We compared static preservation (cold storage, CS, 4 °C) with dynamic preservation (machine perfusion, MP, 20 °C) followed by reperfusion using marginal livers: a model of donation after cardiac death (DCD) livers and two models of fatty livers, the methionine-choline deficient (MCD) diet model, and obese Zucker (fa/fa) rats. CS injury in DCD livers was reversed by an oxygenated washout (OW): h...

Challenges in machine perfusion preservation for liver grafts from donation after circulatory death

Donation after circulatory death (DCD) is a promising solution to the critical shortage of donor graft tissue. Maintaining organ viability after donation until transplantation is essential for optimal graft function and survival. To date, static cold storage is the most widely used form of preservation in clinical practice. However, ischemic damage present in DCD grafts jeopardizes organ viabil...

First Comparison of Hypothermic Oxygenated PErfusion Versus Static Cold Storage of Human Donation After Cardiac Death Liver Transplants: An International-matched Case Analysis.

BACKGROUND Exposure of donor liver grafts to prolonged periods of warm ischemia before procurement causes injuries including intrahepatic cholangiopathy, which may lead to graft loss. Due to unavoidable prolonged ischemic time before procurement in donation after cardiac death (DCD) donation in 1 participating center, each liver graft of this center was pretreated with the new machine perfusion...
